Mild Acne / Comedonal Acne

Few to several Papules/Pustules and No Nodules

BLACKHEADS

  • Small black bumps appear on the skin
  • Affected areas are: nose, center of forehead and chin
 

WHITEHEADS

  • Small, round and white bumps appear on the skin
  • Affected areas are: forehead, areas under the eye and cheeks
 

Treatment

Can consider starting with topical therapy like topical clindamycin or benzoyl perioxide or topical retinoid or combination of both or Azelic acid

What to remember in Comedonal Acne treatments

These are black and heaite heads so mild exfoliation or peeling of skin can be helpful like using topical retnoids or salicyclic acid preparation with proper cleanser

Do's and Don'ts

  • Don't over wash your face! As over-cleansing can cause dryness and inflammation, making your skin feel feel itchy and tender. Usually washing it twice daily is enough.
  • Don't pick, pop, squeeze your pimples as this bad habit can end up with scarring and discoloration
  • Don’t wear heavy make up, as this can clog the pores and induce more zits
  • Don't use exfoliating scrubs too often because they can irritate inflamed skin
  • Don't forget to wash your pillowcase and phone! Dirt and bacteria can transfer from your phone, pillowcase, and anything else that touches your face and clog your pores
  • Don’t use thick moisturizers or oil into your face as this can also clog pores and induce more pimples
  • Don’t go to the Gym or sleep over with your make up on

Moderate Acne / PAPULOPUTULAR Acne

Several to many Papules/Pustules along with Comedones and few to serveral Nodules

 

PAPULES

  • Red bumps with fluid and oil
  • Originate deep inside dermis Pustules
  • Just like a papule but large and engorged
  • Red, swollen and presented as a white pus
 
 

Treatment

For such inflammatory type of acne usually we consider oral antibiotics ideally one of tetracycline group for an average period of 2-3 months along with topical therapy like topical retinoids or benzoyl peroxide.

As Isotritinoin can cause severe dryness of skin and photosensitivity it is important to:

  • Use an appropriate soothing and moisturizing cream
  • Apply sunscreen of SPF 30 and above
  • Use gentle face cleanser
  • Use lip balm that contains SPF
  • Use lubricating eye drops and avoid wearing contact lenses
  • Avoid un necessary surgical procedures while taking this medicine as skin tends to be very sensitive and healing of skin can be delayed
  • Avoid waxing or epilation of hair while taking the medication
  • As medicine can increase blood lipids, try to avoid oily food

  • Do wait 20 to 30 minutes after washing your face to apply acne creams, as this will make your skin feel less sensitive to drying treatments like topical vitamin A
  • Do apply topical acne medications to the entire problem area, not just to the active zits
  • Do eat healthy food! Healthy food and lifestyle always can reflect healthy and clear skin

Severe acne / NODULOCYSTIC acne

Numerous or Extensive Papules/Pustules and many Nodules

  • Occurs in very large numbers, pain
  • Get merge and spread in the entire area
  • Breakouts can occur on squeezing them
 

Treatment

This is considered severe type of acne, and such cases specially after failure of conventional therapy usually oral retinoid (isotritinoin) is indicated for an average period of 6-8 months with proper monitoring of patients blood (liver enzymes and lipid profile) and as it is pregnancy category X, this medicine should not be given to any female whoe is trying to conceive
